Most decision memos bury the recommendation under context and never price the status quo, so the reader cannot actually decide. This forces a 'do nothing' option into the table and a quantified cost-of-inaction section, which is where weak proposals fall apart and strong ones win. It also refuses to invent numbers, listing missing figures openly instead of faking confidence.

How to use it. State the decision in a sentence or three, then dump everything you know, however messy. You get a one-page structure a busy executive can act on. If a number you did not provide would flip the recommendation, it tells you at the bottom instead of guessing.

Worked example. 1. THE ASK (one sentence). What decision you need and from whom, stated so a yes or no is possible.

2. THE PROBLEM (2 to 3 sentences). What is broken or at stake, in business terms, with a number if one exists.

3. OPTIONS CONSIDERED (a short table). At least three rows, and one must be 'Do nothing'. Columns: Option, Rough cost, Main upside, Main risk. Keep each cell to a phrase.

4. COST OF DOING NOTHING. Make the status quo pay for itself. Quantify what continuing as-is costs in money, time, risk, or missed upside. If you cannot quantify it, say what you would need to measure and give a defensible estimate labeled as an estimate.

5. RECOMMENDATION (2 to 4 sentences). One option, why it beats the others given the constraints above, and the first concrete step if approved.

RULES. If WHAT I KNOW is missing a number that would change the recommendation, do not paper over it; list it under 'What would change this' at the very bottom, one line each. Never invent figures; use ranges and label them estimates. Cut any sentence that does not help the reader decide. If the honest recommendation is 'do nothing', say so.
